3 Easy Steps to Refer a Participant to Us
Getting access to the right care is so important. We are passionate about providing care and helping our participants to live their best lives.
If you’d like to get caring support for your participant, here is how you can enlist our help:
Step 1: You complete our form below
We will send you an email to confirm that we have received your referral. Your referral means that you’ve helped connect someone in need with a passionate care team.
Step 2: We conduct an introductory chat
We call the participant (or their family) for an introductory chat. During this call we assess their needs and ensure that we are the right fit for each other.
Step 3: We transform lives
We create a support program personalised to the participant and start providing our care. We love this part, as we are passionate about transforming lives.
Step 1: Send a referral
Our referral form takes just a few minutes to complete, and is the fastest way to get the right support for your participant.
Step 2: Introductory Chat
After we receive your referral, we get in touch with the contact that you have provided. This may be the participant, their family or another contact you have supplied. Generally, we respond to referrals within one business day.
What will happen in this call?
During this call we will:
- Provide more information about our services
- Assess the needs of the participant
- Determine the support required
- Ensure we are the right fit with the participant
- If we connect and want to proceed, we will collect some more information about the participant
Step 3: Passionately transforming lives
When we decide to work together, our team will create a custom, personalised support program to enable the participant to live their best life.
This can include:
- Respite/Short Term Accommodation (STA)
- Supported Independent Living (SIL)
- Support with Daily Living Skills
- Support in Social & Community Participation
- Employment Support
- Children’s Support Services
